  2. the name of your local;
  3. your old address information;
  4. your new address information;
  5. the effective date of the address change, and;
  6. either of:
    a. your employee identification number;
         OR
    b. your CUPW membership number.

  • We will not change your address on our file without the above information.
  • We will notify your local of your change of address.
